APPLICATIONS ARE RETURNED TO APPLICANTS
1. Application is not accompanied by required fee.
2. Application has not been signed.
3. Applicant’s signature does not match the applicant’s
name as listed in part I of the application.
4. Applicant has not provided sufficient information
for the business or home address (A post office box number is not a
sufficient address for Department of State records).
5. Applicant has not completed the municipality information.
6. Applicant required to complete a mandatory education
course has not provided the proof of education certificate.
7. Applicatants name on application appears differently
than on education certificate.
8. Course completion certificate expired. (The education
certificates are only valid for 6 months from the issue date).
9. Application has not been endorsed by the state
senator of the district where the applicant resides or, in the case
of an out of state resident, the state senator of the district in which
the applicant is employed.
10. Applicant does not take the time to carefully
read the application before attempting to complete it.
11. Information is omitted (If omitted intentionally,
this constitutes unsworn falsification of facts made to authorities
and is subject to the penalties of perjury pursuant to 18 Pa. C.S. 4911.)
12. And the number one reason why applications are
rejected is because the applications are ILLEGIBLE!
